Thank you for choosing Advanced Urology Institute. We look forward to working closely with you regarding your urological health care. Patients are seen by appointment at all locations, Monday through Friday. Please contact the office nearest to you to schedule a convenient appointment time.
When your first appointment is scheduled, you will be asked to bring:
- Picture ID
- Current Insurance card(s)
- Previous/current medical records including x-rays
- All of your current medications
- Any referrals required by your insurance plan
Please plan to arrive 10-15 minutes prior to your scheduled appointment time in order to complete any paperwork with our staff. Should you have questions or concerns, please contact the office where you have scheduled your appointment.
From your very first visit, you will experience the difference of Advanced Urology Institute. Please complete the Patient Information Form and bring it to your initial appointment.
